What the request form collects
When you send a request we receive what you type: your name, email address, phone number if you give one, your neighborhood or ZIP code, the options you choose about what you are looking into and when, and any message you write. We also receive the date and time and the page you sent it from.
The form does not ask for medical history, medications, conditions, photos or your date of birth. Please do not include them in your message. If you do, we will pass them on only as part of your request.
Consent
The form has a checkbox you must tick before sending. By ticking it you agree that Lip Filler NYC may share your request with one or more independent providers in your area so they can contact you by email or phone about it. We keep a record that you gave this consent and when.
Who sees your request
The people running Lip Filler NYC read your request and forward it to one or more independent, licensed providers in New York City so that they can answer it. Those providers are separate businesses. Once they have your request, their own privacy practices apply to what they do with it. We do not sell your information, and we do not share it with anyone else except where the law requires us to.
